As part of this mission, Southlake Dermatology was the first dermatology provider in the area to adopt paperless health records, which ensures data accuracy and that a file is never misplaced.
In addition to being a leader in the national healthcare transition to paperless records, Dr. Bowers-Plott embraced the Internet long ago as a tool to improve patient communication. As a result, all new patient forms and paperwork can be found in this section and completed prior to your office visit, which allows Southlake Dermatology staff and physicians to be more efficient and better equipped to help you.
Also contained in this section is our patient privacy notice, which outlines our strict standards for patient confidentiality, as well as the list of insurance providers we accept. Please note that if you do not see your insurance provider on our list, we still welcome you as a patient, but you will be responsible for full payment at the time of your appointment.
